A service animal is defined as a dog that’s individually trained to do work or perform tasks for the benefit of a qualified individual with a disability, including a physical, sensory, psychiatric, intellectual, or other mental disability.

Forms and Advance Notice

  • For reservations booked more than 48 hours prior to travel, Passengers must submit the Department of Transportation Form no later than 48 hours prior to travel. For reservations booked less than 48 hours prior to travel, passengers must submit the completed Form in person to a Customer Service Agent upon arrival at the airport
  • For flights scheduled for 8 hours or more, customers must also submit a DOT Relief Attestation Form, attesting that the dog will not relieve itself in the aircraft or can do so in a sanitary manner
  • Hawaiian Airlines requires service animals to travel with a valid animal health certificate in order to enter Hawaii from outside the state

Travel Requirements

  • Hawaiian Airlines only allows 2 dogs. If you have more, please contact the airline directly
  • Dogs may be quarantined for up to 120 days upon arrival in the State of Hawaii unless they meet all the State’s 5-Day-Or-Less program requirements. (For more information, please visit the link below in the Airline Contact section)
  • Animals will need to remain on the floor and may not obstruct aisles or exit rows
  • Service Animals that cannot be transported safely will not be able fly

Animal Behavior

  • Animals must be trained to behave properly in public and will not be permitted in the cabin if they display any disruptive behavior such as growling, biting, jumping, etc.
  • Animals may not be able to fly if all requirements are not met

International Travel

Please note that not all countries accept animals. You are responsible for any documents and requirements necessary for traveling with animals to your destination country.
